Interventional pain

Pain management billing that holds up on appeal.

Interventional pain claims are denied most often for medical-necessity documentation, frequency limits, incorrect bilateral or distinct-procedure modifiers, and imaging billed on codes that already include it. We fix the coding upstream and appeal what payers deny downstream.

Where it leaks

Three places pain practices lose money.

Medical-necessity and frequency denials

Repeat epidurals, facet blocks and RFA are tied to LCD criteria: documented pain relief, conservative care and session limits. Missing one line in the note is enough to deny.

Bilateral and distinct-procedure modifiers

Modifier 50 lines paid as one side, and 59/X{EPSU} used where it isn't supported, lead to underpayments in one direction and audits in the other.

Imaging billed twice

Most spine injection codes now include fluoroscopic or CT guidance. Billing 77003 on top invites denials; leaving it off where it is separately reportable leaves money behind.

Codes we watch

The lines that decide your reimbursement.

These are the lines we audit first on every pain practice's AR, because they carry the most denials and the most underpayments.

How we work with your team

  • We work inside your PM/EHR (eClinicalWorks, NextGen, AdvancedMD and others) under a HIPAA BAA.
  • We act as a secondary recovery team on aging AR and complex appeals, alongside your staff.
  • Weekly collection reporting to practice leadership.
  • 6–7% of net collections; 10% only on No Surprises Act IDR wins.
FAQ

Interventional pain billing questions

Can we bill 77003 with an epidural injection?

Usually not. Interlaminar (62321, 62323) and transforaminal (64479–64484) epidural codes and facet codes (64490–64495) include imaging guidance in their descriptors, so 77003 is not reported separately with them. Check the current CPT descriptor and NCCI edits for each code.

Why are our RFA claims denied?

Most RFA denials trace back to documentation: payers generally require two diagnostic medial branch blocks with a defined level of relief, plus failed conservative care, before ablation. We check those elements before the claim goes out.

Do you replace our billing staff?

No. We work alongside your team as a secondary recovery unit on aged AR and complex appeals, inside your existing system.
